POLICIES AND PROCEDURES FOR APPLICATION AND ADMISSION
1. Complete Online Application
All applications must be submitted online at www.tads.com.
2. Required Documents
After completing the application linked above, to complete the process you must upload the following documents to the TADS site:
• Copy of birth certificate
• Copy of baptismal certificate, if Catholic
• Teacher recommendation form
• Student report cards for the current year and previous two years for grades 1 through 8
• Copy of any psycho-educational evaluations, if applicable
• Non-refundable $50.00 application fee for grades 1-8, $75.00 for Kindergarten.
• For Kindergarten: Recommendation from preschool, if the child attends preschool Please submit copies only of birth certificate, baptismal certificate, and school records.

Admission Considerations
Students are admitted to Saint Bernadette School consistent with the following considerations:
• Readiness/Academic screenings to determine class placement for new students.
• Immunization records officially certified by a physician or health officer
The order of acceptance for students applying to St. Bernadette School that assess ready for a particular grade is based on the following criteria:
• Children of registered active Saint Bernadette parishioners who have siblings in the school
• Children of Catholic parents active in other parishes who have siblings in the school
• Other students who have siblings in the school
• Children of parishioners active in Saint Bernadette Church. In the event the number of applicants exceeds the number of available openings, the priority will be determined by longevity and activity in the parish.
• Children who are not in the above categories will be admitted on a first-come, first-served basis.
Admission Requirements
• To enter Pre-Kindergarten 4, the child must be four years old by September 1.
• To enter Kindergarten, the child must be five years old by September 1.
• To enter grade 1, the child must be six years old by September 1.
• The birth certificate and baptismal certificate must be presented on the day of registration.
• A non-refundable fee per student must accompany new applications.
• These applications will be accepted in compliance with the admission priorities mentioned above.
• Full disclosure of any medical or educational needs that may impact a child’s learning.
