2. Required Documents

After completing the application linked above, to complete the process you must upload the following documents to the TADS site:

• Copy of birth certificate
• Copy of baptismal certificate (if Catholic) 
• Teacher recommendation(s) (Grades 1-8)
• For PreK 4 and Kindergarten: Recommendation from preschool, if the child attends preschool 
• Student report cards for the current year and previous two years for grades 1 through 8
• Standardized Test Scores (MAP Growth) for grades 2-8
• Copy of any psycho-educational evaluations (if applicable)
• Non-refundable $100.00 application fee for grades 1-8, $125.00 for Kindergarten
